PLEASE NOTE REQUIRED CASE MANGEMENT HH AND HOSPICE EXPERIENCE ON RESUME IN PROFILE ! The professional nurse providing care to medical/surgical patients utilizing all components of the nursing process (assesses, develops, plans, implements, and evaluates) in the plan of care.Services Include:Skilled observation & assessmentWound careInjections & IV therapyNursing Case ManagementNetsmart, formally Allscripts is the EMR.Local Travelers (reside within 50 miles of facility) must upload at a 10% discount of posted rateRequested time off during the course of this assignment MUST be noted and verified at upload. RTO REQUESTS AFTER UPLOAD WILL NOT BE APPROVED NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED (less is better). Non-Billable Orientation: 8hrsHoliday, OT Multipliers: 1.25%
Submission Requirements
Minimum of 2 years CURRENT Home Health experience in Case Management and Hospice Experience RN will be working in the Whitefield/Berlin, NH areaRNs will be providing Home Health & Hospice Case Management services to patients of all ages (infant to elderly)Netsmart/Allscripts EMR experience strongly preferredMileage paid at current approved Federal IRS Rate (Mileage paid from office each morning)Profile Requirements: Work HistorySkills Checklist (pertinent to position), current to 1 year 1 Supervisory Signed Reference (from within the past two years of employment)License/Certifications - Must be uploaded with profile
