Hybrid REMOTE (Work from home with regional travel) The Nurse Practitioner Manager ("NP Manager") leads and manages Nightingale nurse practitioners (NPs) in a designated market or geographical region. This position provides oversight, coaching, supervision and onboarding of NPs to ensure that quality, cost-effective care is provided for chronic kidney disease (CKD) and/or end-stage kidney disease (ESKD) patients. Participates in population health management and overall financial viability of the program. The role functions autonomously, yet in partnership with operational and clinical leadership on accountability for costs, fees, and expenses related to the NPs they manage, quality initiatives; outcomes and performance measurements. Managers care a small patient roster, and flexes within reason, with the needs of the business such as providing coverage for open positions as needed.In this role, you will provide clinical leadership, oversight, coaching, supervision and onboarding of Nurse Practitioners to ensure that quality, cost-effective care is provided for CKD and ESKD patients. You will participate in population health management and overall financial viability of the program. This role functions autonomously, yet in partnership with operational and clinical leadership on budgeting, quality initiatives, outcomes and performance measurements.ESSENTIAL FUNCTIONServe as the clinical lead for a team of NPs, and oversee their work. Ensure NP quality and productivity, including documenting findings, appropriate diagnoses, closing Plan of Care gaps, and accurate coding. Support NP hiring efforts and oversee onboarding of new NPs. Provide ongoing clinical guidance and training to NPs to improve understanding of CKD, ESKD, and commonly associated chronic conditions.Serve as key leader of clinical and operational strategy working with Medical Officer, Operations leadership, NP Education team and providers. Identify opportunities to streamline NP workflow and focus management intensity on high-risk, high-utilization members.Lead complex case reviews and timely support of complex case escalations of high utilization and high-risk members working with a team of specialized resources and patient's care team.Know and comply with DaVita policies and procedures in accordance with State and Federal regulations for patient care and services (CMS, HIPAA, OSHA, or other governing agencies); Standards necessary to maintain organizational accreditation (such as NCQA); and, State or Federal regulations of Injury Prevention and Safety Training ProgramPREFERRED EXPERIENCECA NP license with national credentialing and DEA required.Experience documenting and coding from an MA (Medicare Advantage) risk perspective and conducting Health Risk Assessments on high-risk/chronically ill patients.At least 2 years' experience in supervision or leadership4-6 years of NP Experience; experience in population health, Primary/family care, and/or nephrology a plus.Master's degree in Nursing RequiredPrimary care/family care experience RequiredDialysis and/or Nephrologist experience PreferredExcellent verbal and written communication skillsDemonstrated proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation regarding the needs and care of chronic diseaseDeals with confidential information and/or issues using discretion and judgmentIntermediate computer skills and proficiency in MS Word, Excel, and OutlookHigh level of proficiency with Cerner, Epic or similar EHR systemsExcellent organizational skills and attention to detail.As the Management Service Organization for Nightingale Clinical Care, PC ("NCC"), VillageHealth DM, LLC dba: DaVita IKC ("IKC") assists NCC with recruiting, screening, and interviewing of clinical personnel for employment consideration by NCC.
