Key Responsibilities Conduct comprehensive
patient assessments
to evaluate health status and needs. Develop and implement personalized care plans using clinical guidelines. Administer medications and treatments as prescribed, ensuring adherence to safety protocols. Educate patients and their families on health management and
disease prevention . Monitor patient progress, adjusting care plans as needed for optimal outcomes. Collaborate with multidisciplinary teams, including internal medicine and
critical care transport
specialists. Maintain accurate documentation in electronic health record systems like
eClinicalWorks . Participate in
infection control measures
to maintain a safe environment for patients and staff. Assist with procedures (e.g., dermal fillers) and support patients undergoing
dialysis
as needed. Qualifications
Valid
Registered Nurse (RN) license
in the state of practice. Experience in
family planning ,
critical care transport , or
home community care
is a plus. Proficiency in patient assessment and
infection control
practices. Familiarity with
eClinicalWorks
or similar EHR systems is desirable. Excellent communication and teamwork skills. Compassionate approach with a strong commitment to
patient-centered care
