ResponsibilitiesJOB SUMMARY / PURPOSE

The Supervisor Community Outreach is responsible to the Director of Community Outreach and provides support to St. Vincent’s Hospital Services community outreach services, and is responsible for supervising the work of Community Health Workers within the program.

ESSENTIAL KEY JOB RESPONSIBILITIES

Reviews participant caseload weekly with Director of Community Outreach.

Ensures all forms and documentation are completed accurately and signs completed progress and Outreach forms.

Reviews correct procedures with CHWs as needed in order to correctly complete and document correction efforts.

Completes all Outreach forms as needed and interviews participants if CHW is unavailable.

Tracks Community Outreach data.

Ensures that CHWs are trained on how to properly enter Community Outreach tracking data.

Enters Outreach tracking data of Pathways progress and completion if CHW is unavailable.

Completes Community Outreach reports.

Provides support to CHWs when safety issues arise with Home Visits or participant crises including attending Home Visit with CHW as needed.

Provides resources to support CHWs in work with participants in Community Outreach Program.

Meets with the Director of Community Outreach regularly to receive support and problem-solve.

Completes all required Community Outreach training.

Assists with implementing any Quality Improvement measures that are needed.

Participates in employee recognition and incentives.

Qualifications

Bachelor’s Degree or equivalent in nursing, social work, behavioral or healthcare related field or a combination of education and experience; five (5) years of experience if no degree.

Minimum of two (2) years in social service or healthcare field with focus on population health, and management of care continuum.
