Responsibilities 1. Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.2. Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.3. Observe the following guidelines at each visit:
a. Sign in resident upon entrance.
b. Present resident’s documents to office personnel.4. Before leaving doctor’s office, please observe the following:
a. Obtain resident’s documents.
b. Obtain any new prescriptions.
c. Check to make sure doctor has written on the doctor’s order form.
d. If follow-up appointment necessary, obtain appointment cards.5. After returning to facility, report to RCM and be prepared to answer questions concerning visit.6. Performs other duties as required and assigned.
Qualifications Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
Possess a valid state driver’s license.
Qualified as a Certified Nurse Assistant (C.N.A.).
Clean driving record.
Clean criminal record.
Any other state required certifications
