SUMMARY:
The FSP Clinician is responsible to engage members and to coordinate mental and recovery services that will ensure housing stability and community integration. The FSP Clinician is responsible for maintaining the highest quality provision of direct individual and case management services.
DUTIES and RESPONSIBILITIES:1. Provide, coordinate, and implement mental health and recovery support services with individual members.2. Develop effective, trusting relationships with clients experiencing tri morbid disorders, as well as the forensic population, with a focus on facilitating independence and maintenance of improved physical and mental health3. Collaborate with each member to develop their individual service plan; review and update it upon completion of goals4. Provide linkage to stable housing, and other supportive services as needed to assist clients to obtain housing stability, in addition to client advocacy, case management, and benefit establishment5. Conduct screening interviews, complete housing documentation, and coordinate clients' move-in with the housing authority, property managers6. Perform initial comprehensive case management assessment and develop a plan of intervention that addresses their barriers, increases their income, and assists them in maintaining permanent housing7. Encourage and promote an environment that is strength-based to assist clients in meeting their individual goals8. Identify appropriate permanent housing options for clients, such as, subsidized housing Section 8, Shelter Plus Care, VASH, permanent supportive housing, affordable and market-rate housing, and other housing opportunities9. Work cooperatively and cohesively with other members of the FSP team, including participation in weekly staff meetings, group clinical supervision, and trainings10. Develop and maintain a complete, accurate, and current member file with all funder-required documents
QUALIFICATIONS:
1. MSW or MFT who is BBS eligible (supervision hours are available for this position)2. Must be registered with the board of Behavioral Sciences3. At least one year of experience working with clients with mental illness, chronic health issues, and substance use disorders.4. Demonstrated knowledge of clinical case management5. Knowledge of community resources6. Able to work in a high-tolerance, harm-reduction model with individuals who have multiple barriers7. Strong interpersonal and team-building skills8. Computer literate; able to effectively use computerized database for client file management, with basic skills in Microsoft Word, Outlook, and Excel programs.9. Current, valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record.10. Able to obtain and maintain CPR/1st Aid certification11. Experience working with homeless individuals who are living with mental illness, substance addiction physical health issues, and forensic population preferred12. Minimum 30 WPM typing speed preferred13. Bilingual Spanish preferred
WORK ENVIRONMENT:1. Combination of field and office environment2. Regularly required to sit, stand, bend and occasionally lift or carry up to 35 lbs.3. On occasion walk or drive to different local sites4. Will necessitate working in busy and loud environments5. Will be exposed to elements like cold, heat, dust, noise and odor6.
