Homeless Outreach Coordinator

Summary/Objective
The Homeless Outreach Coordinator will provide situational assessment, crisis consultation, and case management for homeless households or at risk of becoming homeless. They are responsible for completing needs/barriers assessments, developing action plans, facilitating community referrals, determining eligibility, interdisciplinary provider collaboration, and program compliance.
Essential Functions
Participate in public awareness efforts to connect those in crisis with needed resources, including attendance provider collaboratives and community resource fairs.Conduct crisis consults, including client needs assessments, to identify barriers to housing stability and develop action plans that address stability barriers.Function as a trained Coordinated Entry Assessor through South Dakota Housing Development Authority, completing assessments for households needing rapid rehousing or homeless prevention services.Facilitate communication and partnerships with referral agencies and community resources to help clients navigate the support available.Administer housing assistance grant funds as available and appropriate, including eligibility verification, required documentation, payment processing, and overall grant compliance.Facilitate community relationships with hotel managers, property managers, and landlords with available inventory for low-income households, promoting landlord incentives.Assist case management clients with accomplishing their goals through direct support, facilitated referrals, ongoing assessment, and progress reviews.Maintain thorough records of services offered and prepare expenditure reports using the state Homeless Management Integration System (HMIS) and required forms.Work with accounting representatives to prepare and remit required forms and documentation for the monthly grant billing cycle.Provide statistical, expenditure, and progress reports to the South Dakota Housing Development Authority as required by contract and as requested.Participate in local and state collaboratives (Homeless Coalition, Coordinated Entry, Point-in-Time Counts) to enhance the care continuum for those experiencing homelessness.Assist with development efforts to increase donor engagement and sustainability, including program presentations and impact reporting.Other duties as assigned.

This job operates in a professional office environment in combination with community settings. This role routinely uses standard office equipment such as laptop computers, photocopiers, and smartphones. Additional technologies (iPad, radios, clocks, etc.) are in place for supervision, security, safety, and communication.
