Case Management Clinical Liaison
Brookside Inn, a Skilled Nursing Facility, in Castle Rock, CO is seeking a compassionate and experienced Clinical Liaison Nurse to join our team of qualified professionals. In this role, you will be responsible for the case management of short-term residents in a skilled nursing setting. Conduct comprehensive assessments of resident needs and develop individualized care plans.Coordinate with physicians, therapists, and other healthcare providers to optimize care delivery.Monitor resident progress and adjust care plans as needed.Facilitate timely discharge planning and coordinate post-discharge care.Build strong relationships with residents and their families.Communicate effectively with families regarding resident status, care plans, and discharge planning.Address family concerns and provide support and guidance.Clinical Liaison:
Work closes with the admissions coordinator to and help respond to inquiries from healthcare providers and families regarding admissions and referrals.Serve as the primary facility representative facilitating the admission process and procedure for patients and their families, facility staff, and with referral sources.Provide for ease and efficiency in the transfer of the patients to the facility.Coordinate admission inquiries from referral sources and the public, serving as the initial facility representative in inquiring parties for admission; and maintaining relationships with residents and responsible parties.Maintain and develop relationships with referral sources.Complete on-site assessments of potential residents and partners with the interdisciplinary team to assure a smooth transition for new patients.Meet with internal team to educate on hospital needs and ensure proper processes and communication are in place to support admissions.Identify, resolve and/or escalate discharge and admission issues to appropriate teammates for prompt and effective resolution.Ability and willingness to work flexible hours and on-call duties, such as during evenings and weekends, for planned activities or special events, family meetings and conferences, admission calls, admissions, tours, and nurse manager activities.Learn, develop and implement new ways for the company to differentiate itself in the marketplace.Participate actively in regularly scheduled team meetings, conference calls and trainings as needed.Travel to local area hospitals to enhance existing relationships and build new relations with hospital-based teams.Drive strategic sales and marketing initiatives to position the community as a preferred skilled nursing facility.Conduct facility tours and provide information about our services as needed.Actively network with hospital case managers, discharge planners, and other healthcare professionals.Attend professional conferences and networking events.Quality Improvement:
Participate in quality improvement initiatives to enhance patient care.Identify opportunities for process improvement and implement solutions.Collect and analyze data to track performance metrics.Qualifications
Graduate from an accredited school of nursingCurrent, unencumbered, and active license to practice as a Registered Nurse in the state of ColoradoSkilled Nursing Facility experience, 3 years (preferred)Nurse case management experience, 2 years (preferred)Registered Nurse experience, 5 years (preferred)EMR systems, 1 year (Preferred)
