Clinical Care Coordinator

Education: Must possess, as a minimum, a current, active, and unencumbered LPN (Licensed Practical Nurse) license, issued by the Commonwealth of Massachusetts or RN (Registered Nurse). Must be able to read, write speak and understand the English language. Job Purpose / Objective: The primary purpose of this position is to assist the Director of Nursing in planning, organizing, developing and managing the long-term care floor in accordance with current federal, state and local standards, guidelines and regulations that govern this facility, and as may be directed by the Administrator, the Medical Director and/or the Director of Nursing, to ensure that the highest level of practical care is maintained at all times.
