Ideal schedule would be 1st and 3rd week of the month or 2nd and 4th week of the month. Shift time: 8A-4P Any call? No Setting: Outpatient Clinic Duties: Reading studies of all modalities in addition to diagnostic mammograms with breast biopsies Required Procedures: Read Mammo, PET and perform breast biopsies EMR: Cerner Support Staff: Radiology techs, US techs, other support staff Travel Preferences: Preferred Lodging: Preferred hotels Air Travel: Local Please detail any anticipated travel exceptions, for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Credentialing Information: 90-120 days Credentialing with other facilities? No Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less? No Admissions per day: Patients/day: Types of cases: Procedures (wire loc, biopsy, etc): 13, Screening Mammos: 225, Diagnostic Mammograms: 30 Requirements : Active Unrestricted CA License Board Certified/Board Eligible Residency Requirements/Fellowship – Residency Required, No specific fellowship.
