Part Time Social Worker for Inpatient Addiction Recovery Program and Inpatient Med/Surg. Schedule: Fri, Sat/Sun, Mon The Clinical Social Worker is responsible for the assessment and treatment of selected patients and their families in the Inpatient Addiction Recovery (ARP) Service and inpatient medicine/surgery. Reporting to the Social Work Manager or a designated supervisor, the social worker will provide bio-psychosocial assessments and interventions for patients and/or families impacted by a patient’s acute medical needs and any psychosocial factors or barriers impacting their care. The social worker will identify high risk psychosocial factors impacting a patient’s health status and, as an integral member of the interdisciplinary team, will help the hospital staff understand theinfluence of those factors on the patient’s course of medical care. The social worker will effectively intervene through evaluation, emotional support, provision of resources and recommendations, and team collaboration around potential barriers to care in the community and around discharge needs. The social worker will have knowledge in mandated reporting of abuse/neglect, trauma informed care, cultural and/or age specific concerns and impact, unstable housing or homelessness, food insecurity, substance use disorders, psychiatric illness, and the various community agencies and resources available. The social worker will participate in departmental and hospital-wide committees as assigned and will collaborate with other disciplines in the hospital. - Meets with patients daily to provide emotional support, aftercare planning, referrals for programs, and community resources. Identifies patients’ psychosocial, financial, legal, dual diagnoses that effect patient care. - Ensure provision of appropriate, timely, and effective addiction recovery treatment for patients on a Level IV Detox Unit. Employs a range of clinical interventions such as individual, group, and family treatment. - When indicated, provides brief family systems work to provide psychoeducation, emotional support, and guidance around aftercare planning. This includes coordinating family/team meetings as needed, providing emotional support and community resources to patient/family management. - Facilitates hour-long group sessions on various evidence-based topics, including Cognitive Behavioral Therapy (CBT), behavior change, and relapse prevention. - Works effectively as part of the interdisciplinary health care team, communicating regularly with the team and other members on cases and as issues arise. - All work is documented in the patient’s electronic medical record in a timely manner. - Maintains high level of confidentiality for all patients in compliance with HIPAA and Federal guidelines. - Facilitates the appropriate and efficient use of hospital and community resources. - Participates in appropriate hospital wide committees as determined by Social Work Manager such as Acute Care Plan or Safety Committee. - Participates in formal and informal clinical case reviews, clinical supervision, and educational seminars and other tasks as assigned by direct supervisor or Social Work Manager. - Other duties as assigned.
