Certification Details AZ or Compact License, BLS Job Details Occupational Therapist- Please note that traveler must have an AZ license in hand In order to obtain an AZ OT License, the candidate must submit a fingerprint clearance card along with the application. Shift: 5X8s 8am-430pm Daily Caseload per tech: 8-11 patients Years of experience REQ: 1 year First-timers accepted: Yes Weekend REQ: No On Call REQ: No Floating REQ: No Floating REQ to Sister Facilities: No Blocked Scheduling: M-F Locals Accepted: Yes Guaranteed Hours: 40 hours/week, facility is allowed to call off 6x throughout a 13-week assignment. Special Requirements: Outpatient therapy clinic with OT, PT and SLP services. Looking for OT or PT who can support hand therapy in outpatient setting. Case load may not be entirely hand/wrist/upper extremity but this specialty area is a specific need. Additional Preference: Search for support for hand therapy diagnosis and referrals.
