Proposed Schedule: 3 days per week with a possibility of Saturdays. North Creek Health Center Hours of Operation: Mon-Fri 8am-4:30pm, Sat 9am-12pm.
Anticipated start date for this position is 12/1/2024
Position Summary
The Licensed Practical Nurse provides age appropriate, population specific, patient care in a professional, compassionate, caring, culturally and ethnically sensitive manner, while embracing the Core Values of Quality, Appreciation, Creativity and Sustainability in pursuing the HHHN Mission and Vision. The LPN administers care to patients under the direction of the responsible Registered Nurse or medical provider. To perform this job successfully, an individual must be able to perform each duty satisfactorily.
Essential Duties and Responsibilities:
- Serve as a mentor and resource for nursing standards, competencies, policies and procedures.
- Collaborates with staff and other departments in the development of strategies to continuously improve quality of care and the patient experience.
- Utilize data to continuously meet care coordination needs across settings in conjunction with the patient, family and/or other care providers.
- Provide direct and indirect care that is safe, ethical, patient and family-centered, and evidence-based.
- Protect patients and employees by adhering to infection control policies and protocols.
- Prepare the patient for encounter by following established protocols.
- Demonstrate competence in clinical skill set.
- Demonstrate proper medication and vaccine management processes.
- Reconcile the medication list and prepares prescription renewals.
- Demonstrate complete, accurate, and timely documentation in the electronic medical record.
- Respond to patient phone calls and engage provider or RN appropriately.
- Reinforce patient and family education plans that meet the needs of the patient.
- Perform ordered point of care testing (CLIA), including specimen collection, and proper documentation.
- Assist with patient encounter flow and trouble-shooting issues in collaboration with supervisor.
- Provide guidance and mentors new and incumbent staff based on skill set.
- Attend meetings and serves on various organization committees.
- Assist supervisor with compliance and staff development duties.
- Assist with inventory and receipt of medical supplies, pharmaceuticals, and equipment.
- Complete health center daily, weekly or monthly tasks as assigned.
- Ensure operation of medical equipment by following manufacturer’s instructions, troubleshooting malfunctions, and requesting repairs.
- Perform additional duties as assigned.
Qualifications: The requirements listed below are representative of the knowledge, skill and ability to perform the essential functions:
- Unrestricted current LPN license to practice nursing in New York State.
- Must possess a certificate from an accredited school of nursing.
- Minimum of 1-year clinical experience preferred.
- Ability to travel throughout the Network.
- Current BLS certification.
- Proficiency with Microsoft applications and electronic medical records.
- Excellent verbal and written communication skills.
- Self-directed, detail-oriented, motivated and ability to work independently as well as in a team atmosphere.

The pay rate for this position is $28.00/hour.
