What Will Your Job Look Like?
The
Mobility Assessment Evaluator (MAE)
is responsible for reviewing ADA paratransit application materials, conducting in-person interviews, and performing functional assessments with applicants to determine eligibility for ADA paratransit service. The MAE is also responsible for maintaining accurate documentation and providing final eligibility determinations to the Client.
This position will sit in our Honolulu, HI office. The schedule is Monday – Friday 8am-5pm.
What You’ll Do:
Determine final eligibility based on the application, interview, functional assessment, and any supplemental information.
Conduct in-person interviews to assess mobility capabilities.
Determine required functional assessments for applicants.
Provide written eligibility determination documentation to Client within ADA-required and contracted time frames.
Read and interpret ADA paratransit applications and verification forms.
Review interview results and application paperwork, and follow up on assessments as needed.
Maintain ongoing communication with the Quality Assurance team and management regarding contractual deadlines.
Assist applicants with completing documents and scanning into the Applicant’s case as needed.
Conduct functional physical and/or cognitive assessments, which may include:
Tinetti Balance & Gait Assessment
Physical Functional Assessment using an indoor and outdoor course
MMSE
FACTS
Input all information into Client software (Trapeze) and other software as appropriate.
Participate in staff meetings and community outreach as needed.
Complete all necessary reports and maintain ongoing applicant tracking logs.
Assist with conducting phone calls to applicants and providers.
Maintain a positive attitude when interacting with individuals and caregivers.
Conform to MTM’s HIPAA standards.
What You’ll Need:
Bachelor’s degree in Social Services, Rehabilitation, Ancillary Health Care Profession, or a related field.
3+ years of professional experience working with individuals with disabilities.
Experience with the Americans with Disabilities Act (ADA) regulations.
Related experience with functional assessments of individuals with disabilities.
Even Better If You Have...
Experience conducting information-gathering relating to ADA Paratransit eligibility.
Solid interviewing, assessment, and observational skills.
Ability to be trained in administering standardized evaluation tools.
Intermediate proficiency with Microsoft Office Suite.
Ability to maintain a high level of confidentiality.
