Job Description:

" Traveler must meet all minimum requirements below to be considered.
" Do not submit traveler who have or will have expiring licenses and/or certifications during time frame of entire assignment. Traveler will automatically be declined.
" Only submit traveler who can work schedule in posting and interested in the unit.
We are seeking a Phlebotomist to:
" Perform phlebotomy procedures on patients of all ages. .
" Ensure proper patient identification and specimen labeling.
" Complete specimen receiving and specimen processing.
" Patient check-in and order entry of laboratory tests in the laboratory information system.
Minimum Requirements:
" Facility Requirement:
o Traveler must have permanent address outside 50 mile radius of Madison (permanent address must be included in traveler's profile).
" Experience:
o Minimum two years experience in phlebotomy required
o Pediatric and Epic experience required.
o Prior traveler experience required.
" Licensure and Certification:
o Phlebotomist (American Society for Clinical Pathology) certification preferred
o BLS required must be certified through American Heart Association ONLY
" Education:
" High school diploma or equivalent (education verification is required)
" References
o Needs 2 completed references from recent work environment/assignments (supervisor or manager only for work performed in the last two years).
" Floating
o Travelers are the first to float to floors working with children up to 1 year of age and/or based on the traveler's competency skills/experience. The Traveler may have shifts scheduled in multiple units, depending on the current staffing needs and/or different locations within the organization. During the traveler s assignment, if the primary unit no longer has a need, shifts will be re-assigned to another unit in order to complete contract.
Location: UW Health Clinics in Dane County and surrounding areas - this is a float position
Parking: Free parking onsite at all clinics.

Shifts:
40 hrs/week, Days, 8 hour shifts as needed by location. Start times will vary between 7:45 am and 9:00 am, Monday - Friday. This traveler will float to UW Clinics in Dane Count and surrounding areas. 26 week assignment.
