Summary: Direct Care Worker members will provide direct care services to the client(s) in their care. He/She is responsible for maintaining the day-to-day activities of the client’s home and community activities.
Essential Duties
- Become familiar with the client's medical needs, medications, diagnoses, strengths and limitations, etc.
- Monitor, support, and follow daily schedules and routines
- Follow Individual Service Habilitation Plan goals and objectives
- Discuss with the client, as necessary, issues regarding problem solving, information sharing, and crisis intervention
- Assist client in maintaining a clean and orderly home environment.
- Assist client with independent living skills (i.e. budgeting, shopping, cooking, cleaning, hygiene, laundry, etc.)
- Accompany the client into the community to engage in age/developmentally appropriate activities
- Develop a schedule/calendar of activities for the clients
- Complete daily progress notes and service information in the client Communication Book
- Maintain open line of communication with the team members (client, other staff, supervisor, and case manager)
- Attend team meetings to provide input on organizational, behavioral, academic issues, etc.
- Consult with Supervisor each week about any outstanding issues that need to be addressed
- Follow the Positive Behavior Plans of clients who have a plan created and collect data as appropriate
- Assist client with administering medication
- Ensure medication is in a lock box so that the client does not have access
- Ensure client is administered the appropriate dosage
- Complete Incident Reports once an incident occurs and submit to Supervisor immediately
Qualifications & Requirements:

- Must have a valid Driver’s License
- Must have reliable transportation
- Must have and maintain current liability auto insurance
- Must have a High School diploma or GED and/or 2 years’ experience working with individuals with disabilities
- Must have good communication skills (oral and written)
- Must have adequate analytical and decision-making skills
- Must possess knowledge regarding the needs of persons with disabilities
- Must possess the ability to work with others and manage crises
- Must be reliable and dependable
- Must be trustworthy
- Must maintain confidentiality
- Must have good customer service skills
Other Requirements
- Maintain 12 hours of training in "Best Practices" for working with individuals who are Developmentally Disabled
- Attend CPR/First Aid Training
