Well Care Hospice is a family-owned and operated organization dedicated to providing exceptional end of life care in the Triad area. This up and coming Hospice program has had a phenomenal first year, making strides to empower patients to live life on their own terms. Despite COVID-19 our Well Care Hospice team has continued to make visits as scheduled and as needed with each and every patient and family to ensure their comfort, support, and safety. At Well Care Health the number one guiding principle is, "Patients come first," and Well Care Hospice is looking for a Registered Nurse who lives that same principle and is eager to be an integral part of our growing Hospice team and program. Our current service area includes: Davie County, Davidson County, Forsyth County, Southern Stokes County, and Yadkin County. Up to $10,000 Sign On Bonus!
Well Care Hospice RN Case Managers are responsible for:
Identifying and anticipating needs and coordinating the care provided to the Hospice "unit of care" (patient, family, caregivers) from admission to discharge
Providing direct nursing care in the patient's home, wherever the patient calls home, under the direction of the Director and under physician orders
Collaborating with other Interdisciplinary Team members, including the physician, social worker, chaplain, hospice aide, and volunteer, to ensure the provision of exceptional care quality and symptom management
Ensuring patient, family, and caregiver confidence and competence through ongoing education regarding disease processes, symptoms associated with illness, and interventions used to maintain comfort and safety at the end of life
Providing on-call nursing services 24/7 (including weekends and holidays) throughout our service area - rotating schedule with other RN Case Managers
Required Qualifications:
Graduate of an accredited nursing program
At least 1 year of nursing experience
Current North Carolina license as a Registered Nurse
Current CPR certification for healthcare workers through the American Heart Association
Current driver's license, proof of acceptable automobile insurance coverage, and reliable transportation
Excellent communication skills, both written and orally
Exceptional ability to develop strong interpersonal relationships with the hospice interdisciplinary team, patients, families, caregivers, and referral sources
Strong computer skills, especially with Microsoft Office and Electronic Medical Records
Preferred Qualifications:
Experience in hospice or home health
Certification in Hospice and Palliative Care (CHPN)
