Responsibilities:

• Perform hospice social work visits in assisted living, independent living communities, skilled nursing facilities, and residences for hospice patients.
• Conduct comprehensive assessments of patients' social and emotional needs
• Develop personalized care plans in collaboration with the interdisciplinary team
• Provide counseling and support to patients and families facing end-of-life challenges
• Assist with coordination of care and access to community resources
• Maintain detailed and accurate documentation of patient interactions and progress
• Utilize Homecare Homebase EMR (knowledge of which is a plus)

• On-call rotations with weekend work as needed
• Mileage reimbursement also paid at .53/mile

Requirements:

Qualifications:

• Licensed as an LMSW
• Minimum of one year of hospice care experience
• Minimum of one year of social work experience
• Strong communication and interpersonal skills
• Compassionate and empathetic approach to working with the elderly
• Ability to manage a fast-paced, independent work schedule
• Knowledge of Homecare Home Base EMR is a plus
