Reporting to the Manager of RN Liaisons, the RN Liaison facilitates referrals to hospice, assists in
developing relationships with business partners in the community, and performs timely, individualized, accurate assessments for patients referred to the organization. The RN will represent GHS in a professional manner and is committed to delivering the highest quality of care possible as identified in the GHS mission statement.

Alternating weekends; West Pasco county Hospitals.

EDUCATION AND QUALIFICATIONS:

  • Currently licensed in the State of Florida as a Registered Nurse
  • A minimum of two (2) years clinical experience working in an acute care settings; one year of homecare and/or hospice experience preferred.
  • Active CPR Certification
  • Must maintain current Florida driver’s license
  • Must maintain valid automobile insurance coverage

ESSENTIAL JOB RESPONSIBILITIES:

  • Maintains professional and clinical knowledge of provider services. Acts as liaison between referral partners and agency to communicate information about new products, programs, and service delivery.
  • Makes initial contact and physical, social, and psychological and financial assessment of patients to determine appropriateness of hospice services.
  • Meets with patients and families to discuss hospice services, individual needs/concerns, and manage expectations; obtains signed consent documents.
  • Effectively communicates with patients, family, supervisor, and physicians in facilitating a smooth admission.
  • Initiates the patient’s plan of care and educates patients and families on the services offered by GHS.
  • Successfully builds relationships with case managers, social workers, physicians etc.
  • Works with referral partners to provide appropriate alternatives for patients who do not meet hospice criteria including patients with no funding source.
  • Provides ongoing assessments of physical, psychological, social, and spiritual needs of the patient/family unit and initiates appropriate intervention; documentation is accurate and returned to Intake Coordinators within 24 hours.
  • Anticipates future needs of the patients and is pro-active in ordering appropriate DME and requesting nursing/CNA/Volunteer Services.
  • Serves as hospice resource to hospital partners.
  • Maintain a working familiarity with admission criteria, and any other pertinent Hospice issues. Interprets hospice policies and acts as resource for case managers, physicians, hospital and community for effective public relations and marketing of services.
  • Works cooperatively with other Patient Access Team members to provide information about GHS services and other appropriate community resources to address the verbalized need for assistance.
  • Initiates new patient medications, equipment, supplies and transportation upon admission as needed.
  • Demonstrates clinical skills regarding case management and application of GHS policies and procedures such as: intravenous skills, pain management, symptom management, and patient/family relationships.
  • Adheres to GHS clinical and general policies and procedures.
  • Handles patient problems related to unmet expectations for hospice.
  • Effectively communicates with physicians who are non-supporters of organization’s hospice services.
  • Conducts individual and group presentations on products and services in an informative, accurate, and professional manner. Responds to questions about products and services.
  • Keeps the agency informed of facility changes regarding bed capacity, staff changes, new facility programs and/or procedures.
  • Educates the community regarding Hospice care, philosophy and concept through a case-by-case consultation with individuals in the community, educational in-services or seminars when requested.
  • Possess the ability to multi-task and exhibits adaptability and flexibility in maintaining compliance in correlation with GHS standards.
  • Ensures all patient care and documentation of care is compliant with GHS standards, Florida State and Federal Regulations.
  • Ensures professional standards of Hospice nursing practice are maintained.
  • Serves as a professional resource to the community.
  • Rotating holiday coverage as assigned, minimum of one holiday shift per year.
  • Other duties as assigned.
