ABOUT CASCADE:
Cascade Community Healthcare (CCH) has been providing treatment services to Lewis County and surrounding areas for more than 50 years. Our mission is to provide our clients with compassionate and encouraging counseling leading to a renewed sense of hope and the ability to make positive changes and live more satisfying lives. Cascade currently has six treatment locations in Lewis County.
We are looking for a Registered Nurse (RN) or a Licensed Practical Nurse (LPN) to join our Outpatient Medical Team. You will provide medical support to enrolled clients and providing treatment coordination and consultation with primary clinicians and case managers.
Schedule:
Monday - Friday 8:00a - 5:00p
This position will normally require that the employee work a standard, Monday through Friday, forty (40) hour work week. This position is classified as being “non-exempt” according to the Fair Labor Standards Act.
PAY SCALE:
LPN: 26.25 - $32.00 (DOE)
RN: $40.25 - $49.06 (DOE and Degree)
QUALIFICATIONS:
- Must have Washington State Department of Health Registered Nurse (RN) credential OR Licensed Practical Nurse (LPN) credential.
- Current BLS certification.
- Two years’ experience working with individuals with sever and persistent mental health disorders preferred.
- Must be able to pass WA State Patrol background check. A national background check may be required if have not lived in WA state consecutively the last three years.
- Previous experience working as a member of a multidisciplinary team and working collaboratively with other service providers preferred.
- Experience in medical, mental health or social services field preferred.
- Must be able to represent the agency in a positive and professional manner.
- Must be able to communicate clearly, both verbally and in writing.
- Must be able to enter information into a computer network system.
- Must comply with agency policies and procedures.
- Must maintain current contact information and mailing address on file with the human resources office.
WHAT YOU'LL DO:
- Provide services to enrolled clients within scope of practice/credential.
- Review orders from medical providers.
- Coordinate treatment services with agency programs, other relevant agencies and others involved in the client’s life, i.e., family, friends, employer and others the client may choose.
- Meets direct service productivity requirements as assigned by program supervisor.
- Maintains records and reports in accordance with agency policies and procedures.
- Participates in supervision, consultation and training at the direction of the supervisor.
- Coordinate client data processing procedures and provides consultation to Cascade staff regarding patient care as needed
