Community Bridges is looking for full-time or part-time Nurse- RN!

We are a non-profit agency founded in 1982 with a mission to advance the integration, growth, and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access to find creative solutions to develop meaningful community engagement.


The Nurse- RN in coordination with the Senior Nurse Trainer will be responsible for the oversight and direction of individuals' medical needs, including training for staff as appropriate. Responsible for development and monitoring of practices that safeguard the health and well-being of the individuals and employees that we support. Responsible to ensure compliance with He-M 1201 regulations and other regulations as designated by the state and to provide the primary point of contact for clinical questions as they relate to DHHS regulations.


Qualifications:

  • License: Registered Nurse in NH and possess a current, unencumbered license.
  • Experience: At least two years of licensed experience within the past 5 years. At least one year of experience supporting mental health, developmental disabilities, and/or acquired brain injuries within the last five years.

Compensation and Benefits

There is a $1000 sign-on bonus for this position - $500 will be paid at the time of hire and $500 will be paid after 6 months in the role. *Please note, sign-on bonuses are for new hires only. Re-hires and transfers are not eligible at this time.

Paid Time Off: For a full-time employee working 40 hours a week, 264.16 hours of Paid Time Off is received per year! PTO days will increase over the years based on longevity and there is no waiting period to start using your PTO!

Mileage Reimbursement: Employees are reimbursed for approved mileage driven on the clock.

Paid Training: As an employee, you are paid your usual rate of pay while attending training!

Flexible Hours


Specific Duties and Responsibilities:

  • Complete training provided for Administration of Medication to others.
  • Develop and recommend policies and procedures to safeguard the health, safety and well-being of supported individuals and employees.
  • Collaborate with various medical professionals and team members to improve the quality of medical supports as necessary.
  • Assisting in the completion of written reports and analysis, in compliance with the Healthcare Coordination and Administration of Medication regulations, and submit to the State in accordance with He-M 1201 and other regulations as designated by the state.
  • Provide ongoing support as necessary to individuals, employees, and medical professionals. When appropriate, provides in person support for critical health appointments, events, or needs.
  • Provide ongoing oversight and training to staff related to individual's specific healthcare needs.
  • Provide staff training, coordination and develop the necessary medical protocols to support individuals.
  • Ensure availability of remote nursing consultation 24-7-365, through oversight of on-call nursing phone rotation.
  • Perform additional duties and responsibilities as assigned.

Required Skills:

  • Must be a licensed as a registered nurse in New Hampshire and possess a current, unencumbered license.
  • Demonstrate competence to teach adult learners through completion of He-M 1201 Nurse Trainer program conducted by the Bureau Developmental Services, DHHS.
  • Ability to communicate thoughts and ideas well in both verbal and written formats in either group or one-to-one settings.
  • Ability to represent Community Choices services to the public and maintain a positive image to the community at large.
  • Ability to work as a member of a team and carry out team decisions.
  • Ability to collaborate with individual families, team members, and the medical professionals on behalf of each individual.
  • Ability to assist and teach on personal care, providing personal care if needed.
  • Must have knowledge of and demonstrate through personal actions Community Choices mission, values, policy and procedures.
  • Must be proficient in computer competencies such as Outlook, Microsoft Word, Microsoft Excel and the ability to learn agency software to perform functions of the role.
  • Ability to work a flexible work schedule (such as respond to the support schedules as needed for trainings, assessment/observation, etc.). Hours may include some evenings, early mornings or occasional weekend hours.

Work Environment and Physical Demands:

  • Work Environment- can be occasionally hectic with occasional periods of high stress. Occasional to moderate short deadlines. Must be willing to manage a schedule that is flexible and responsive.
  • Physical Demands- must be physically able to lift 50 lbs and to provide transfers as necessary. Walking and standing are required frequently with occasional stair climbing.

Travel:

Travel required within the state frequently. Must have access to personal vehicle during all scheduled work hours, maintain valid driver's license and appropriate insurance, and uphold an acceptable driving record. May be required to transport individuals and families receiving services on occasion.
