We are hiring Homecare RN or LPN's Per Diem (weekends required) in Lowell and the surrounding areas. We care for mental health patients of all ages and backgrounds who require daily visits in their homes.

Position 1: RN or LPN Per Diem to work Weekends, Weekdays AM and PM shifts in Lowell and the surrounding areas, up to $35/visit.

Are you the RN or LPN we're looking for?

To thrive as a Home Care Nurse at Comfort Home Care in Lowell, you'll need a unique blend of skills that keeps our clients' health and happiness at the forefront. Whether you're a Registered Nurse or Licensed Practical Nurse, you'll perform thorough initial and ongoing assessments to identify each client's medical needs and level of functioning. Flexing your problem-solving muscles is essential as you initiate and revise care plans while conducting OASIS assessments at key moments. Effective communication is vital; you'll coordinate services and keep physicians and other team members informed of any changes in a client's condition. Additionally, collaboration is key-working hand-in-hand with patients, families, and other healthcare providers will ensure comprehensive planning for care and discharge.

With your ability to prepare clinical notes and submit paperwork swiftly, you'll turn every home visit into an opportunity for excellence and compassion!

Knowledge and skills required for the position are:

  • RN or LPN provides initial and ongoing assessments to identify the client's medical needs level of functioning and ability to communicate and learn.
  • RN or LPN initiates the plan of care necessary revisions and performs OASIS assessments at appropriate time points when applicable.
  • RN or LPN initiates and performs appropriate nursing interventions.
  • RN or LPN coordinates services informs the physician and other personnel of changes in the client's condition and needs.
  • RN or LPN collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
  • RN or LPN prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.
