RN Case Manager Details and Compensation:

• Location: 100% hybrid
• Payrate: $38 - $44 per hour
• Required Availability: Monday - Friday 8am - 5pm

RN Case Manager Responsibilities:

• Case management for children medical services (0-age 21)

• Tier 4 & 3 members

• Completing telephonic outreach monthly or quarterly

• Home visits upon member enrollment, quarterly for tier 3 members.

• Attending meetings, meetings for members, participation in regional meetings and activities that are required by the team

• All documentation is completed through TruCare, Teams, Zoom.

RN Case Manager Requirements:

• Pediatric case management / pediatric nursing

• TruCare

• Community Care

• Medicaid experience (AHCA)

• Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2 – 4 years of related experience.
