POSITION SUMMARY:

Conduct a comprehensive assessment of patient biomedical, psychological, social and functional needs.

Engage patient as part of a care team through advocacy, ongoing communication, health education, identification of resources and service facilitation.

Utilizes professional judgment, critical thinking, motivational interviewing and self-management techniques to assist patients in overcoming barriers to goal achievement.

Collaborate with appropriate physicians, clinical pharmacist, social workers, nurses, therapist and community health workers to co-manage patients with complex medical and social needs.

Perform outbound contacts to patients approved by the multidisciplinary team.

Document services and outcomes in patient plan of care. Participate in weekly meetings to address patient outcomes.

Participate in the quality improvement and evaluation process with activities designed to reduce risk, inclusive of data collection, analysis and follow-up interventions.

Adheres to the SWCS code of ethics, and complies with local, state and federal laws and the mental health codes.

Serves on agency committees, working groups and other projects as assigned.

Performs other duties as assigned by supervisor, contractual obligations and/or program description.
ESSENTIAL JOB REQUIREMENTS:
EDUCATION:
Associate Degree in Nursing (ADN), Bachelor of Science in Nursing (BSN), or diploma from an accredited and approved nursing program.
EXPERIENCE:
Two (2) years of nursing experience
Case Management experience preferable
REQUIRED
CERTIFICATIONS & LICENSURES:
Registered Nurse or Licensed Practical Nurse with a valid, unrestricted State of Michigan license. Possession of a valid driver’s license and access to a private vehicle for day-to-day job performance.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
Computer skills and knowledge including Windows, Microsoft Word, and Microsoft Excel.
Ability to communicate effectively in writing and verbally.
Demonstrate the ability to triage and apply critical thinking skills.
Understanding of community resources and the needs of diverse populations.
HELPFUL SKILLS:
Creative and resourceful with good negotiating skills preferred
Experience in discharge planning, home health care, rehabilitation medicine, community health or managed care preferred.
PHYSICAL REQUIREMENTS:
Working Conditions
Services will be provided in the office, the clients’ homes or other appropriate community settings. Work may require travel in an urban community.
Physical Requirements
Sitting, standing, walking, driving, repeated hand and wrist motions (for use of computers, phones, and other office equipment) are required. Must be able to tolerate varied weather conditions when traveling within the community.
