Job Title: Hospice RN Case Manager


Tranquil Oaks Hospice Care is currently seeking a full-time hospice RN case manager to join our team to admit and lead the coordination of patient care with the multi-disciplinary team and in compliance with Tranquil Oaks policies and procedures. The hospice RN case manager is an integral component of our dedicated and compassionate team of nursing and hospice professionals. At Tranquil Oaks Hospice Care, patients always come first.

JOB SUMMARY:


  • The Hospice RN Case Manager is an experienced and professional registered nurse who initiates and coordinates the hospice care plan, provides skilled and palliative care and coordinates the services provided to his/her patients.
  • The RN works cooperatively with the Attending Physician, Hospice Medical Director, and other members of the interdisciplinary hospice team.
  • The RN participates in patient care conferences and supervises the Home Health Aides assigned.
  • The RN provides care, as described in this position description, to patients who have been diagnosed with a terminal illness.
  • The RN regularly reviews and updates the plan of care.
  • Performs prescribed medical treatments, including pain management and symptom control, conducts assessments and evaluations, provides education and supportive care to patient and caregiver.


DUTIES AND RESPONSIBILITIES:


  • Possesses an understanding of the hospice program and benefits.
  • Knowledge of the principles of the intake, admission, and transition to care process with the ability to further develop skills in these competency areas.
  • Evaluate patients for hospice appropriateness, e.g. medical records review as requested.
  • Develop a detailed Plan of Care for assigned patients ensuring all services needed are identified and met while working closely with the Interdisciplinary Group, patient, family and Attending Physician.
  • Provide comprehensive nursing evaluations and assessments.
  • Provide field supervision to LVNs and Home Health Aides assigned to your patients.
  • Documents assessment, current medications and initiates appropriate care plan.
  • Responsible for assessing patient’s clinical status, planning, teaching and implementing immediate interventions on the day of admission.
  • Coordinator with Attending Physician, Hospice Medical Director regarding patient admission and follow up needs.
  • Communicates admissions information, issues, and concerns in patient care meetings to assure smooth transition and continuity of care with other assigned staff, including a detailed report to the assigned Case Manager and assigned Social Worker.
  • Performs routine nursing visits when required and serves as a backup to other field staff and Director of Patient Care Services as requested.
  • Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient’s family has concerning hospice care.
  • The Case Manager is available to do admissions at the patients’ home, hospital, board and care or skilled nursing facility.
  • Coordinates medications and DME needs for same-day delivery.
  • Responsibilities will include but not limited to performs comprehensive nursing evaluations, demonstrates flexibility in scheduling and readily accepts new admissions and emergency assignments as requested, participates in QAPI program as required, adheres to the highest standards of professional and personal conduct in the performance of his/her duties.


QUALIFICATIONS:


  • Graduate from an accredited school of nursing. Baccalaureate degree in nursing from a program accredited by the National League for Nursing preferred.
  • Currently licensed as a registered nurse to practice in the State of California.
  • Minimum 1 year of experience as a professional nurse within the last three years.
  • Previous experience in hospice, home health, admissions preferred.
  • Current CPR/BLS
  • Understands the philosophy of hospice concepts. Experience with the terminally ill patient.
  • Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.
  • Ability to handle multiple priorities, documentation requirements and deadlines.
  • Ability to organize and prioritize workload independently.
  • Must possess excellent interpersonal skills particularly with family cultural/sensitivity.
  • Demonstrated ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in a nursing environment.
  • Has excellent observation, verbal and written communication skills.
  • Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
  • Proficient computer skills required using specialized software, and equipment used in the delivery of patient care.
  • Committed to improvement, clinical excellence, high-degree of patient and staff satisfaction.
  • Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing to perform required nursing skills.
  • Must have reliable automobile, valid driver’s license, and the minimum state required liability auto insurance.
  • Able to travel to multiple locations in the course of the working day/week.
  • Demonstrates excellence in customer service and time management.
