JOB SUMMARY

Under the direct supervision of the Operations Director, the PACE Home Care Coordinator - RN is responsible for the development and implementation of homecare services for program participants, including the coordination of all Durable Medical Equipment (DME) and community-based care.

REQUIREMENTS

1. Bachelor of Science in Nursing.
2. Current RN license in the state in which the PACE center is located.
3. At least 1 year RN with home or community health provider experience.
4. Supervisory experience desired.
5. 1 year of experience working with a frail or elderly population preferred. If this is not present, training on working with a frail or elderly population will be provided upon hiring.
6. Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
7. Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
8. Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
