Job Summary: 
The Hospice Quality Director is a key part of the management team at St. Gabriel's Hospice & Palliative Care, responsible for managing and coordinating the agency's QAPI Plan, Risk Management, Infection Control, and education programs to ensure compliance with Federal and State regulations. Serving as an educator, the Quality Director assesses staff educational needs and develops programs based on clinical requirements and interests. They integrate hospice regulatory and evidence-based best practice knowledge when orienting staff or providing direct care. Responsibilities include data analysis for trending, process improvement, and utilization review. 

Responsibilities:
  • Oversee the quality assurance performance improvement program, including HQRP data and reporting, process and outcome reports, occurrence reports, hospice compare and patient satisfaction data, program quality monitoring data, utilization data, payer inquiry requests, denial activity, fall data, and infection control practices.
  • Use data trends to initiate and sustain performance improvement activities.
  • Serve as a regulatory resource for Federal and State regulations and standards.
  • Develop, monitor, and evaluate corrective action plans for deficiencies identified through quality review and survey processes.
  • Handle chart workflow processing and review of documentation for accurate submission of HQRP data to CMS in line with Conditions of Participation and billing.
  • Compile data reports for monthly audits to ensure compliance with agency policies, state, and federal regulations.
  • Oversee Fall and Infection Control programs, recommend performance improvements to reduce adverse events and rehospitalization data.
  • Plan and coordinate special classes, conferences, and meetings to enhance staff performance and quality of care.
  • Fulfill compliance responsibilities related to the position.
  • Develop, conduct, and evaluate programs to meet educational needs of home care clinical staff.
  • Assist in the development, implementation, interpretation, and evaluation of training programs.
  • Utilize learning technologies to develop, launch, and monitor instructor-led, in-service, and online learning.
  • Collaborate with nursing leadership to evaluate staff performance post-educational initiatives.
  • Conduct field supervisory visits with clinical staff to ensure a competent workforce.
  • Perform other duties as required.
 
Qualifications/Requirements:
  • Experience: A minimum of two years in a healthcare-related setting required; hospice experience preferred.
  • Education: Graduate of an accredited school of nursing required.
  • Licenses/Certifications: Current Texas RN license required; valid driver's license.
  • Other: Understanding of hospice care regulations and Conditions of Participation.
