Job Summary

Under the direction and supervision of the Director of Nursing Services, the RN Assessment/MDS Coordinator is responsible for conducting and coordinating the completion of the resident comprehensive assessment which includes the Minimum Data Set (MDS). The process includes the coordination of appropriate participating health professionals (interdisciplinary team) for the purpose of conducting initial and periodical comprehensive, accurate assessments of each resident to plan care that allows the resident to reach his/her highest practicable level of physical and mental and psychosocial functioning.

Essential Functions

Any and all job-related duties and responsibilities as directed by management including:

1. Clinical

Using good judgment, conducts and coordinates the comprehensive assessment of each resident’s medical, functional and psychosocial needs which includes but is not limited to:

  • Ensures that initial and periodic assessments be completed with coordination and appropriate participation of health professionals
  • Schedules of members of the interdisciplinary team to review resident assessments for accuracy and updating accordingly
  • Monitors plans of care of residents to ensure resident assessments and care plans present an accurate reflection of resident’s physical, mental and psychosocial functioning
  • Evaluates and identifies resident outcome to determine if residents are achieving their highest practicable level of well-being
  • Keeps accurate record of time frames for review assessment (MDS log), which includes quarterly and yearly review, significant changes and care plan updating.
2. Administrative

Develops and maintains a flow of communication that enhances the expected positive resident outcome, includes but is not limited to the following:

  • Supports the overall goals of the facility and company
  • Maintains standards of practice for resident assessment
  • Ensures that all residents are assessed promptly and timely
  • Ensures exchange of essential information necessary for the accurate completion of resident assessments
  • Ensures that all documentation necessary for comprehensive assessment is maintained according to Federal, State and company policy
3. Supervision

Responsible for supervision of nursing personnel as it relates to resident assessment by direct supervision or delegation per Director of Nursing Services, which includes but is not limited to the following:

  • Establishes Nursing Services goals and coordinating the interdisciplinary team approach to resident assessment
  • Coordinates MDS assessment meetings to review resident assessment tool and ensuring its accuracy
  • Coordinates care plan meetings to assess needs of residents based on comprehensive assessment
  • Conducts record reviews and staff meetings per DON as it relates to resident assessment in order to provide input and feedback on facility provision of quality of care
  • Manages and supervises the work of the MDS staff
4. Consumer Service

Works with Administrator, all levels of Nursing personnel, physicians, consultants, Department Heads, residents, resident’s family or legal representative, and other disciplines determined by resident needs.

Responsible for nursing personnel’s adherence to healthcare industry and company standards in the area of consumer service.

  • Presents professional image to consumer through dress, behavior and speech
  • Adheres to company standards of resolving consumer concerns/complaints
  • Ensures that all resident consumer rights are protected
Additional Responsibilities & Duties

Treats all information and data within the scope of the position with confidentiality and security

Cooperates fully in all risk management activities and investigations

Physical Demands & Working Conditions
  1. Work is performed primarily in a standard office environment with frequent interruptions
  2. Ability and mobility to work in an office setting; to stand or sit for prolonged periods of time; to occasionally stoop, bend, kneel, crouch, reach, and twist
  3. Lift, carry, push, and/or pull light to moderate amounts of weight up to 20lbs
  4. Operate office equipment requiring repetitive hand movement and fine coordination including use of computer keyboard
  5. Must follow all safety measures and procedures
  6. Must follow the dress code policy
Required Qualifications
  1. Current license as a Registered Nurse in the State of California
  2. Graduate from an accredited School of Nursing
  3. Some experience in MDS or related field
  4. Knowledge of current Federal and State requirements for assessing resident and developing care Additional Requirements
  5. Must keep all required certificates and/or licenses current
  6. Strong administrative and organizational skills
  7. Ability to coordinate different levels of appropriate health professionals to assess the care needs of residents appropriately based on Minimum Data Set and develops a comprehensive care plan
  8. Ability to work independently, or with minimal input and supervision to carry out the vital tasks of assessment and coordination
  9. Leadership ability and capacity to work effectively with all levels of personnel
  10. Able to communicate effectively verbally and in writing (English)
  11. Able to adequately perform the physical requirements of the position
  12. Knowledge of Title 22 and OBRA regulations
