With a staff complement of 200 strong, we serve a radius of 100 miles around Missoula with a broad array of professional healthcare including nursing, occupational therapy, physical therapy, speech therapy, social work and bereavement support. Our programs encompass Home Health, Hospice (the largest Hospice in the community), Home Infusion, Home Medical Equipment (including home modifications for those who want to age-in-place) and HCBS (the Medicaid “waiver” program that supports those who wish to live with independence).


This is a Full-Time position with a business hour visit schedule and a rotating schedule for on-call and holiday coverage. We use a patient triage service for on-call support which handles most patient need during the on-call shift. Part-Time position may be available for the following:


  • RN Case Manager
  • RN Visiting
  • LPN

Responsibilities

Our RNs and LPNs...put the patient at the center of care... They work with a inter-disciplinary team to provide skilled nursing care, and case management for patients in their homes. RN-Visiting and LPN's communicate needs to the RN Case Manager to ensure continuity of care for patients and their families. They work with a staff of professional healthcare providers including Home Health Aides, Social Workers, Chaplin's, and other team members. Through patient and family instruction and evaluation, they provide the highest quality care and attention. They maintain accurate, current and complete patient records to assist with patient outcomes. 


Qualifications

Required: A current RN or LPN license in the State of Montana, a valid driver’s license, appropriate auto insurance, and reliable transportation. 

Preferred: 2 years of recent acute care or home care experience preferred.


This position requires Montana Nurses Association union membership for RN's working out of the Missoula office.

 

Schedule:

Specific work days and work hours are as directed by your manager. RNs participate in the on-call schedule; LPNs do not. On-call scheduling is assisted through a first-contact service that we contract with to minimize the disruption in our nurses lives; the service reduces the number of calls by approximately 50%.
