What is PACE North? PACE stands for Program of All Inclusive Care for the Elderly. We are a new and unique option for individuals 55 and older who qualify for nursing facility level of care and wish to remain in their homes as they age. What does that mean? We care for the elderly daily both at our state-of-the-art day center on Garfield near Hammond during regular business hours and as needed in their home. Never heard of us before. That’s okay, we’re new to the region. We began serving the elderly in 2019 and our service model is so popular it’s growing quickly!

What does an RN Case Manager at PACE North do? What’s different about being an RN Case Manager at PACE North? Great question! At PACE North the RN Case Manager is a care decision maker as part of our interdisciplinary team! What does that mean? Instead of simply following the care instructions, you’re an equal decision-making partner and can help shape the care plan! You’ll also lead and monitor staff who provide care, evaluate outcomes, and build relationships with both participants and their family members. This role conducts initial and periodic assessments and obtains and monitors supplies and equipment. In this role you’ll be visiting participant homes and participating in our remote on-call rotation. Finally, you'll be able to provide PACE North participants with the most cheerful and professional leadership they've ever received!


Requirements:

What qualifications should you bring to the table? We’re looking for that special person who loves serving others with a positive approach. You should have a current, valid RN license, and a high school diploma or equivalent, but we’d prefer an ADN or BSN. We’d also like to see 2 years of comparable relevant experience in a home or community health setting. We’re also looking for success working in a team environment. In a key coordination role like this, it’s also critical to demonstrate strong interpersonal communication, decision-making, and problem-solving skills. You should also have a valid driver license. Most importantly, we’re looking for someone with an inspirational, motivational, and creative approach!
