Job Overview:
The Care Coordinator will work closely with patients, healthcare providers, and administrative staff to ensure efficient patient management and high-quality patient care. This position involves managing patient schedules, coordinating care, and providing support to patients throughout their treatment journey.
Key Responsibilities:
• Act as the primary point of contact for patients, addressing inquiries and providing support
• Manage and coordinate patient appointments, ensuring timely and appropriate scheduling
• Communicate effectively with medical staff, patients, and families regarding care plans
• Assist with insurance verifications and pre-authorizations as needed
• Maintain patient records and ensure compliance with relevant regulations
• Support patients in navigating healthcare services and accessing community resources
• Promote patient education regarding treatment options and health management
• Collaborate with physicians and other healthcare professionals to coordinate care
Qualifications:
• Bachelor's degree in healthcare administration, nursing, or related field preferred
• Prior experience in a healthcare setting, preferably in care coordination or related roles
• Strong organizational and multitasking skills
• Excellent communication and interpersonal skills
• Ability to work collaboratively with diverse teams and adapt to a fast-paced work environment
Preferred Skills:
• Knowledge of orthopedic procedures and terminology
• Familiarity with electronic health records (EHR) systems
• Proficiency in Microsoft Office Suite and scheduling software
• Bilingual skills are a plus
