We currently have multiple openings for NYS licensed Social Workers (SW) to work full-time (FT) as part of our United Hospice team. The positions are in-person, with a salary range of $60k - $80k and is commensurate with education, applicable experience, licensing and certifications.

Location Serving both Orange and Rockland Counties in NY
Schedule: Day Shift, Monday to Friday, 8:30 am - 4:30 pm (includes a weekend rotation of 1-2 weekends/month)
Responsibilities:
- Contributes to patient and family counseling and education
- Participates in ongoing interdisciplinary comprehensive assessment
- Assesses patient/family psychosocial response to the terminal illness and evaluates coping skills
- Assists patients, family, and agency staff in utilizing community resources
- Assists team members in understanding the significant social and emotional factors related to the care of the terminally ill patient and family
- Coordinates with team members to plan and implement the hospice plan of care
- Social Worker will participate in interdisciplinary team meetings
- Social Worker maintains accurate, thorough and professional written records, and charts relevant information in a timely fashion, following UH documentation standards
- Participates in and is actively involved with the bereavement portion of the program as appropriate
- Incorporates organization’s commitment to customer service and continuous quality improvement into daily work habits. This includes promoting positive co-worker and teamwork relationships and supporting the goals and mission as set forth by the agency
- Accepts additional responsibilities as assigned
Qualifications:
- MSW from a graduate school of social work is required. Licensed by the State of New York
- LMSW required, LCSW a plus
- Two years minimum of supervised experience in the health field or two years of experience in family therapy/counseling
- Ability to relate to a diverse population and workforce in a personable and effective manner
- Ability to relate to the terminally ill and their families; bereavement experience preferred
- Proficient in MS Office Suite, EMRs and able to learn new software systems
- Daily access to a vehicle, a valid driver's license/acceptable driving record and insurance coverage for daily travel to patient locations
