Make a Difference in the Lives of Frail Seniors

We're seeking a passionate and dedicated Social Worker to join our interdisciplinary team (IDT). In this role, you'll provide comprehensive primary care and social services to help our senior participants live safely and independently in the community.

Your Responsibilities:

• Conduct psychosocial assessments and develop personalized care plans.

• Deliver social work interventions, including resource arrangement, counseling, and case management.

• Collaborate with the care team to ensure smooth transitions between care settings.

• Initiate and facilitate care conference meetings with participants, families, and caregivers.

• Provide discharge planning and support.

Qualifications:

• Master's Degree in Social Work (MSW).

• Minimum of one year of experience working with a frail or elderly population.

• Experience coordinating care conference meetings.

• Experience with behavioral health and substance abuse issues is preferred.
