Hours:

Monday to Thursday 9am-6pm

Friday 9am-3pm

Position Overview:

  • Evaluate and Support Patient Needs: Based on medical provider referrals, conduct brief screenings to assess the mental health and support needs of patients. Perform comprehensive bio-psychosocial assessments to identify behavioral health, substance abuse, co-occurring disorders, domestic violence, and medical needs, followed by appropriate follow-up care.
  • Integrated Treatment Planning: Provide early interventions, referrals to specialty behavioral health services, and collaborate on integrated treatment plans that help clients achieve their goals. Participate in daily primary care huddles to identify opportunities for intervention.
  • Deliver Behavioral Health Services: Offer behavioral health services, including low-intensity/brief treatment, Motivational Interviewing, Problem-Solving therapy, and Behavioral Activation, delivered in a culturally, linguistically, and age-appropriate manner. Maintain productivity expectations of 8-10 consultations per clinical day, including psycho-education, medication adherence counseling, disease self-management counseling, clinical services self-care plan development, and patient follow-up.
  • Coordinate Care: Support and coordinate care with the patient's primary care provider and other behavioral health providers. Act as the behavioral health specialist within the primary care team, providing educational resources and participating in ongoing training in adult/child behavioral health issues.
  • Crisis Intervention and Community Collaboration: Be immediately available during working hours for psychiatric emergencies. Develop and maintain collaborative working relationships with community providers and establish a current listing of community linkages to meet the needs of the target population.

Minimum Qualifications:

  • Licensed Mental Health Practitioner (LMSW, LCSW, LMHC, PhD, PsyD, LMFT)
  • Experience in or a desire to learn about patient education and/or chronic disease management
  • Ability to effectively engage clients in a therapeutic relationship
  • Ability to work with clients through telehealth and in person
  • Experience with screening and assessment for common mental health/substance use disorders
  • Comfort with the pace of primary care and working with an interdisciplinary team
  • Strong communication skills
  • Ability to implement clinical pathways and protocols for treatment of selected chronic conditions

Preferred Qualifications:

  • Yiddish language competency

Pay & Benefits:

  • 50/hr-75/hr for LMSW commensurate with experience or 75/hr-125/hr for LCSW commensurate with experience
