Job Description
This position exists to facilitate post acute referrals to statewide home care agencies or extended care facilities. The position will be a liaison between Throughput Services and the post-acute facilities, ensuring that patient/family needs are communicated in a timely and efficient manner. This position will work closely with agencies in and out of the Care Alliance Services network of quality providers to ensure services are established with minimal delay or impact to patient care. High School Diploma/GED is required. Requires 3-5 years of relevant experience. Requires knowledge of payor practices and/or guidelines. Requires experience in a clinical setting and familiarity with medical vocabulary. Requires experience managing and prioritizing multiple tasks under critical and/or time -sensitive situations. Requires experience working with a widely diverse population of customers and clinicians. Requires experience with databases or spreadsheets. About Us
