POSITION SUMMARY/RESPONSIBILITIES
This position will serve as the link between our Women’s Health patient population and the community; health system providers and staff; CareLink; and inpatient and outpatient service lines to improve utilization of Healthy Texas Women (HTW) and maximize the use of existing health services as appropriate. Assists and guides patients through community health prevention and treatment services to ensure that patients are able to access and keep eligibility and medical appointments, participate in recommended care plans, and receive appropriate health information. Assists and supports the marketing efforts of the Health Promotion Department in community venues including, but not limited to, health fairs, University Health sponsored events, educational classes provided within the Women’s Health service lines. All functions and behaviors are applicable to the clinical, educational, and community settings.
EDUCATION/EXPERIENCE
Bachelor’s degree in social sciences, health education or related field of study is required. Four years of experience in the area of health education, public health, social work and/or case management may be considered as a substitute in lieu of the Bachelor’s degree. English/Spanish fluency is preferred.
