Job Description

PURPOSE OF POSITION

Under supervision of Nursing Leadership, provides professional nursing care to patients/clients within an assigned unit. Diagnoses

and treats responses to actual or potential health problems by means of nursing methods and techniques

such as case-finding, health teaching, health counseling and provision of care supportive to and restorative

of life and well-being using an interdisciplinary team approach,

Nursing Care -Med-Surge Units

Summary Of Duties And Responsibilities

  • Admit patient and complete admission assessment and history.
  • Formulates nursing diagnosis based on admission history, physical assessment and observation of patient need.
  • Assesses patients who are at risk for falls and skin breakdown using the risk assessment tool.
  • Assess patient dietary and social needs.
  • Develops a plan of care based on admission assessment including identified learning and psychosocial needs.
  • Sets goals that are measurable and realistic.
  • Sets the above mutually with patient and family.
  • Address discharge planning needs in the plan of care.
  • Establishes priorities in addressing medical and nursing diagnosis including safety measures for at risk patients.
  • Provides nursing interventions as determined in the nursing care plan.
  • Responds promptly to symptoms or changes in patient status exercising reasonable judgment.
  • Transcribes physician orders accurately.
  • Administers medications and treatments in a safe knowledgeable manner following the Department of Nursing/Patient Care Services policy on medication administration.
  • Administers special treatment procedures within the scope of nursing practice and hospital policy.
  • Assists in other diagnostic and therapeutic procedures within the scope of nursing practice and hospital policy.
  • Implements and records patient/family teaching.
  • Participates in and implements patient discharge plans.
  • Evaluates nursing plans at the start and end of duty.
  • Update nursing care plans frequently based in changes in patient's medical condition.
  • Evaluates and records patient response to treatment, medication, teaching and other nursing interventions performed on patient.
  • Communicates pertinent patient information to Head Nurse or Assistant Head Nurse and Supervisor, co-workers, physicians and other health team members as appropriate.
  • Proficient in the use of electrocardiogram equipment

Minimum Qualifications

  • A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

Special Note

For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, must hold or obtain through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
