LPN (Long –Term Care) – Eddy Village Green (EVG) – Cohoes, NYIf you are looking for a LPN position in skilled nursing, in a unique style of care, this could be your opportunity. Here at St. Peter's Health Partner's, we care for more people in more places.This is a 192 bed skilled nursing facility providing care in a non-traditional way through 16 twelve-bed houses. Greenhouse is a pioneering effort in long-term care bringing control back to the elders that live in the houses by honoring their lifelong routines and preferences. In addition to the culture change in care delivery there is also a relationship shift between nurses and the C.N.A's that manage the individual houses.Central to The Green House® Project's mission is the development of Green House homes—small-scale, self-contained, and self-sufficient nursing home and assisted living settings that put elders at the center. Each home includes private rooms and bathrooms for each elder, a living room with a fireplace, and outdoor spaces that are easy to access and navigate.Position Highlights:Top quality care: Rated as one of Newsweek's Top Nursing Homes for 2021 and Tine Union's Best Nursing Home for 2021!Unique campus: With a campus that is more like "grandma's house" EVG is not your traditional 1 building nursing home and is the largest of the three in NY state with this model!Commitment to safety: Through Trinity Health's distribution network PPE has never ran out and staff has never had to reuse PPEComprehensive orientation: Due to the unique campus, leadership wants to ensure staff is trained and ready for the floor so orientations typically last 6 weeks compared to the standard 4Quality of Life: Where career opportunities and quality of life convergeAdvancement: Strong orientation program, tuition assistance and career developmentFlexible schedule: Leadership tries to be as flexible with scheduling as possible, but looking for 7pm-7am 36 hours a week.Engaged leadership: Leaders that help on the floor in the homes and commit to the development and needs of their staffClose to home: Location that is an easy commute from Albany, Latham, Troy and Clifton Park!What you will do:Administers nursing care to residents under the direction of a Registered Professional Nurse. Assists with treatments and procedures and carries out selected duties within the framework of the established nursing plan of careResponsibilities:In collaboration with the RN collects data about residents and determines priorities is resident care based upon identified needs and goals.Contributes to the plan of care through collaboration and effective communication with the RN and other members of the healthcare team.Under the direction of the RN and within the scope of practice, complies with established nursing policies, procedures and standards to provide safe, quality care to residents.Administers medications and monitors IV therapy as ordered and according to established policies and standards.Provides emotional support to residents and their familiesWill go between houses on campus to provide care and pass medications to residentsWhat you will need:Required:A current license to practice as a Licensed Practical Nurse (LPN) in the State of New York6 month of experience or more as a LPNPay Range:$24.00 - $30.10 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
