The COMPASS Nurse Care Manager works as part of an interdisciplinary team to coordinate care for HIV positive patients by addressing barriers to treatment adherence, viral load suppression and retention in care. The COMPASS Nurse Care Manager performs clinical nursing duties for HIV positive patients who present for medical visits, including triaging patients, getting vitals from patients, administering medications and performing bloodwork.
RESPONSIBILITIES:
Meet with HIV positive patients when they present for medical visits
Perform any relevant clinical nursing duties needed for patient care
Screen patients for mental health and substance use treatment needs, refers for needed services
Works closely with medical providers, nursing staff and other Institute employees to coordinate care for patients , through formal and informal case conferences, huddles , warm hand-offs and joint visits
Provides individual and group based health promotion, treatment adherence counseling, risk reduction counseling and case management to assist patients in decreasing barriers to viral load suppression and retention in care
Regularly review health maintenance topics and outreach patients as needed for compliance
Complete Prior Authorizations to ensure timely access to needed medications
Coordinate with pharmacies on behalf of patients
Participate in initial assessment of service needs and service barriers; assists in developing a comprehensive individualized service plan; follows up with patient to ensure service plan implementation.
Valid NYS nursing license; RN required
Computer literacy with proficiency in MS Word, Excel, and PowerPoint.
Demonstrated organizational, interpersonal, oral and written communication skills and the ability to handle multiple assignments at any time
Ability to generate, interpret and analyze data from multiple sources
Bi-lingual, Spanish preferred
