Job purpose Under general supervision, the Nursing Supervisor - RN Case Manager works closely with Providers, Outreach & Enrollment, Behavioral Health and Clinical Staff as a key member of the Seward Community Health Center (SCHC) Care Team by identifying and coordinating health maintenance needs for patients and supervising a team of Medical Assistants and other Care Team Support Staff to ensure the delivery of high-quality patient care and promote optimal health outcomes in our community. Duties and responsibilities The following examples of duties and accountabilities illustrate the general range of tasks but are not intended to define the limits of the required duties. Case Management: * Assists patients in gaining access to services, coordination of care, oversight of their entire case, and linkage to appropriate services. * Assists primary care providers in developing individualized integrated care plans for patients, making referrals, and monitoring and evaluating care and referrals. * Promotes patient awareness of health and wellness issues and provide resources to manage their conditions through one-on-one patient meetings as well as the execution of health education classes, workshops, conferences, community presentations, and attendance at outreach events. * Provides informational resources to aid patients gaining access to medical, social, educational and other services in the community * Refers patients to the appropriate providers to assist in the coordination of care. * Conducts a thorough chart review of scheduled patients for all providers in advance of the appointment to identify health maintenance and best practice deficiencies, orders POCT or labs to assist in meeting identified deficiencies in accordance with standing orders, assure referrals have been done and notes have been received, and reviews notes from outside offices to get a general idea of the plan of care. Tracks and conducts patient follow up calls for all patients seen in the clinic or the ER. * Supports the Clinic Manager with the implementation of continuous improvement activities for clinical operations, including use of outcome data to support and direct programs in establishing goals and achieving improved results. * Supports the Quality Assurance (QA) and Risk Management Programs of SCHC, including development, maintenance, and refinement of related policies & procedures, self-audits, and care monitoring systems. Lead clinical committees, as assigned. Clinic Nursing: * Serves as a Clinic Nurse. Completes nurse visits including medication and immunization administration. Assist with procedures and medical assessment of patients as appropriate in conjunction with providers. * Triages patient phone calls, requests and messages using the designated communication systems. Prioritize for specific action needed for provider intervention or contact, medication refills, provide for available appointment slots, etc. Handle and refer emergencies appropriately, including crisis intervention and triage. * Oversee patient flow, monitor waiting room, exam room availability, provider schedule and information and patient flow between the front office staff and clinical support staff. Ensure smooth flow of patient visits with minimum waiting time and high levels of patient satisfaction. Review provider schedules for appropriate scheduling times. Staff Management: * Serve as first point of contact for questions from Care Team Support Staff. * Hires, orients, trains, supervises, monitors, and evaluates staff including maintaining adequate level of staffing and ensuring all duties are being completed according to SCHC policies and procedures. * Develops workflows to support Care Team Support Staff ensuring that goals and objectives of the team are properly defined, streamlined and clearly established. * Creates and fosters a teamwork environment. Identifies potential problem areas, develops a system for objectively monitoring performance and creatively seek solutions to foster quality improvement initiatives. * Establishes performance indicators regarding customer service standards and models expected behaviors that demonstrate service excellence with a focus on patient care. Qualifications Education, Licensure & Experience (all levels) * Alaska RN license required. * Minimum of one year of supervisory experience required. * Minimum of four years of nursing experience required. Primary care or FQHC environments preferred. Experience Level I: 4-5 years' experience as a licensed RN. Experience Level 2: 5-6 years' experience as a licensed RN. Experience Level 3: 6+ years' experience as a licensed RN. Other (all levels): Pre-employment drug screening, TB test, required vaccinations per policy, current BLS certification, background check and reference check.
