Care and Connections for Families provides in-home counseling and support services to families whose children are experiencing a mental health crisis. Sheppard Pratt provides crisis intervention services that address the family’s immediate needs. We also help pave the way toward a long-term, stable family environment with support services.

What to expect.

We are seeking a licensed social worker or counselor to provide mental health services. You will work closely with in-home stabilizers to develop and implement service plans for youth and their families from Central and South America who are experiencing a mental health crisis.

Specific responsibilities include:

  • Providing clinical assessments and treatment services to assigned clients and their families.
  • Maintaining at least the required minimum case load (10 clients) as interpreted by our county contract monitor.
  • Completing all required documentation in a timely manner and ensuring maintenance of complete clinical record as directed by policies and procedures.
What we need from you.
  • Must possess one of the following at time of hire:
    • Licensed Certificated Social Worker – Clinical (LCSW-C)
    • Licensed Masters Social Worker (LMSW)
    • Licensed Graduate Professional Counselor (LGPC)
    • Licensed Clinical Professional Counselor (LCPC)
    • Licensed Marriage and Family Therapist (LMFT)
    • Licensed Clinical Marriage and Family Therapist (LCMFT)
  • A driver’s license with 3-points or less and access to an insured vehicle.
  • Experience working with at-risk adolescents and families is preferred.

The pay range for this position is $62,400 minimum to $87,838 maximum. Pay for this position is determined on a number of factors, including but not limited to, license and years and level of related experience.
