Overview

Responsibilities:

As an outpatient palliative care advanced practice provider, you will provide compassionate and holistic care to patients with serious or life-limiting illnesses and their families. You will work collaboratively with a multidisciplinary team of physicians, nurses, social workers, chaplains, and other specialists to address the physical, emotional, spiritual, and social needs of patients and caregivers. A candidate for this position will:

  • Perform comprehensive assessments
  • Develop and implement individualized care plans, prescribe medications
  • Order and interpret diagnostic tests, and provide education and counseling
  • You will also participate in quality improvement initiatives, research, and education activities related to palliative care.
  • Care will be provided at patients home

Qualifications

REQUIRED QUALIFICATIONS

  • Master’s degree or higher in nursing or physician assistant studies from an accredited program
  • Current Illinois license as a nurse practitioner or physician assistant
  • Certification by the American Nurses Credentialing Center (ANCC) or the National Commission on Certification of Physician Assistants (NCCPA)
  • Minimum of two years of experience as an APP in primary care or a related field is preferred
  • Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications
  • Proficiency in electronic medical record (EMR) systems
