Job Description

Job Duties:

The Nurse Practitioner, in conjunction with the team leader, has overall clinical responsibility for monitoring recipient treatment and staff delivery of clinical services. The nurse practitioner provides psychiatric and medical assessment and treatment; clinical supervision, education, and training of the team; development, maintenance, and supervision of medication and administration and psychiatric and medical treatment and procedures. In addition, having FNP (Family Nurse Practitioner) experience is necessary in areas which includes Pediatric Primary Care, Adult Primary and OBGYN.

Summary Of Duties And Responsibilities

The following responsibilities are unique to the Nurse Practitioner position:

Along with the Team Leader, treatment planning, general healthcare, medical and psychosocial approaches.

Collaborates with each nurse practitioner and/or clinical nurse specialist, in assessment, treatment planning, general healthcare, medical and psychosocial approaches, and a review client's progress and treatment. Provides consultation and training regarding the client's medical psychiatric care, including pharmacologic management needs.

Works with consumers and staff in developing and implementing person centered individual treatment plans which address psychiatric issues.

Educate consumers and their families regarding medications/symptoms/illness/side effects.

Provide on-site crisis assessment and management and collaborate with acute and long-term inpatient providers.

Collaborates with other providers as necessary (i.e., primary care physicians).

All team members are generally responsible for the following activities:

Participates in the ongoing assessment and therapeutic for clients.

Completes, as directed, elements of the comprehensive assessment for client within 30 days of admission and uses in collaboration with team during the treatment planning meeting.

Works patient in engaging and motivating clients to achieve their treatment goals.

Assists clients maintain financial entitlements.

Attend team meetings when on duty in order to facilitate an integrated, multidisciplinary team approach to service:

Daily Team Meeting- to organize work for each day and to review recent contact with each client (attend at least weekly).

Treatment planning Meeting-weekly or bi-weekly to develop and review treatment plans/decisions. Maintains compliance with all applicable practice standards and guidelines.

Maintains client confidentiality and adherence to HIPP A requirement at all times.

Completes all required documentation in a timely manner consistent with agency guidelines.

Maintains agency required productivity standards.

With documented consent, engages the client's family and/or significant others to provide education regarding mental illness and system resources.

  • While any one staff member may have the primary responsibility to serve in a particular capacity, as the need arises, any one staff member can be asked to do or assist in completing a task within the limits of licensing, certification and skill requirements.

Attendance/Tardiness/Excessive Sick Leave

If you call out sick the day before or after a pass day, weekends, holidays you will be required to submit a note from your physician (see attached memos): Annual Leave, Call in Procedure, Excessive Sick Calls)

Minimum Qualifications

  • A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
  • Current certificate as a Nurse Practitioner issued by the NYSED; and
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
