This position will promote, maintain and improve individual’s health to patients in an outpatient setting at AIDS Services Center. This position will provide high-quality nutritional care and chronic disease management and prevention counseling. May also serve as a resource to assist individuals, other professionals or the community.
JOB DUTIES AND RESPONSIBILITIES:
- Serves as a dietitian/health coach for all designated group of patients. Duties may include initiating contact, assisting with care navigation, connecting to programs and services, and following through to ensure compliance.
- Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication to treated patients.
- Nutritional assessments and routine follow up/ reassessment of patients per nutritional standards of care, nutritional complexity and protocols/ pathways.
- Maintains appropriate, timely and effective communication with patients.
- Provides individuals or group counseling and education to patients when necessary.
- Identifies barriers, develops a treatment plan, and provides assistance and support to patients based on their needs relating to health education programs.
- Engages and maintains constructive relationships within diverse population.
- Acts as a resource in resolving any questions or concerns pertaining to health risk or status.
- Acts as a resource and change agent by influencing attitudes, modifying behavior and introducing innovative strategies to improve patients’ health status.
- Demonstrates an understanding of cultural and social barriers
- Monitors health education programs effectiveness.
- Maintains complete and accurate documentation on all patients to facilitate follow up and program data reporting. Assures complete data collection and recording in corresponding database.
PHYSICAL AND SENSORY REQUIREMENTS:
Sit up to 6 hours per day. 4 hours at a time. Stands for up to 8 hours per day; 6 hours at a time. Walk 6 hours per day; 10 minutes at a time. Frequently fingering, handling, firm grasping. Frequent twisting and turning. Occasionally lift, carry, and push objects up to 20 pounds. Frequently stoop and bend. Frequently reach above shoulder level. Must be able to hear as it relates to normal conversation.
EDUCATION:
Active dietetic registration through Successful completion of a Didactic Program in Dietetics (DPD) or Coordinated Program in Dietetics (CPD) plus Dietetic Internship (DI) accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) and for Pennsylvania: PA state license maintained through PA state nursing board. Received certificate of training applicable to job responsibilities must be obtained within 1 year of application.
Bilingual Spanish preferred
TRAINING AND EXPERIENCE:
Active dietetic registration, and for Pennsylvania: PA state license through PA state nursing. Minimum 4 years of experience
