Responsibilities

  • Job Responsibilities: This position represents a unique opportunity to provide integrated mental health services to patients in an outpatient setting. The Licensed Clinical Social Worker (LCSW) works independently as well as collaboratively to evaluate and address patient's mental health needs in a sophisticated and innovative healthcare model. The LCSW is a member of a multidisciplinary care team that collaborates to meet the patient's and family's medical and mental health needs. The Licensed Clinical Social Worker conducts psychiatric diagnostic assessments and thorough psychosocial assessments, creates a behavioral health treatment plan, and utilizes evidence-based practice interventions to achieve improved medical and mental health outcomes. The LCSW is responsible for timely completion of documentation for billing of services and coordination of care. The LCSW is also responsible for managing the emergent mental health needs in the clinic such as, but not limited to, acute anxiety, depression, and crisis interventions. The LCSW will demonstrate strong clinical skills in assessment and treatment for mental health issues along with advanced triage, critical-thinking, and problem-solving skills in a clinic setting. Advocating for patients and families, analyzing psychosocial needs, and resource coordination are also expected in this role. The incumbent will work directly with other members of the healthcare team to address, counsel, and educate around behavioral health factors impacting barriers to improve overall health outcomes for patients.
Qualifications

Required:
  • Two years post graduate
  • Master Degree Social Work
  • Licensed Clinical Social Worker (KY)
Desired:
  • One year as a social worker
