Other Info: * Number of Office/Clinic Patients Per Locum Per Day: 20 * Number of Hospital Based Patients Per Locum Per Day: 12 * Physicians in Your Group? 13 * Number of Patients Per Day: 15 * Level I Trauma Center * Required Procedures: Esophageal Dilatation/Manometry, Esophageal Manometry Interpretation, Small Bowel Biopsy, Lower GI Endoscopy, Sigmoidoscopy, Colonoscopy, Insertion/Management of Sengstaken/Blakemore Tube, Capsule Deployment, Bravo, PH impedance, Breath test Interpretation, Esophageal and Anorectal Manometry Interpretation and Endoflip. * Estimated 3 month credentialing timeframe SUBMISSION REQUIREMENTS : * IL license at the time of submission - Highly Preferred due to IL licensing delays * Must have proof of COVID-19 Vaccine - Required at time of name clear * Provider must be able to perform therapeutic procedures such as ERCP and EUS. - Required at time of name clear CERTIFICATION REQUIREMENTS Board Certified - Required EMR System: Epic
