Overview:

  • Mendota Health is a leading provider of mobile wound care services.
  • We specialize in helping patients heal their wounds by providing the best possible treatment options in the comfort of their own home.
  • This is an excellent opportunity for a new graduate or experienced NP to join a growing organization whose mission is to change how wound care treatment is delivered and increase access to high quality wound care.

This position a is full-time salaried position with the flexibility to work Monday-Friday. No weekends, evenings, on-call or holidays!

Daily local travel is required to patient's homes. All mileage is reimbursed. Equipment, medical supplies, and training will be provided.

Qualifications

  • Graduate of an accredited nurse practitioner or physician assistant program
  • A clinical background in adult, family, geriatric, podiatry, or vascular specialties
  • At least one year as an advanced practice provider
  • Board Certified/Prepared through AANP, ANCC or NCCPA
  • Wound care experience a plus
  • Nurse Practitioner national certification as ANP, FNP, or GNP required or ability to obtain
  • Current, unrestricted license in the state of assignment or the ability to obtain license
  • Ability to obtain and/or maintain State DEA licensure / Prescriptive Authority post - where applicable
  • Microsoft office and CRM experience preferred

Practice Details

  • The PA or NP is an experienced member of the health care team who provides advanced wound care to patients in their home setting.
  • Assesses health status for wound care patient populations using advanced practice knowledge and skills.
  • Develops and implements an individualized patient plan of care including coordination of care between visits.
  • Thorough Assessment: Conduct comprehensive patient assessments, paying special attention to factors that increase the risk of wound formation.
  • Advanced treatments like surgical debridement, UltraMIST therapy, and skin substitutes.
  • Medication Management: Order necessary medications and provide clear instructions for their use.
  • Educate and Communicate: Educate nursing staff, and maintain open communication with primary care physicians, families, and clinical teams as required.
  • Contribute to effective and efficient practice management policies and practices that support patient access to medical care.
  • Proven ability to function at highest level within scope of practice.
  • Patient education and pro-active disease management experience.
  • Demonstrates ability to provide care or service adjusting approaches to reflect developmental level of population served.
  • This individual must show commitment to exemplary patient care.
