Providence ElderPlace is seeking a full-time Geriatric Medicine physician to join its team at Hillsboro. This is a single, coordinated program that provides comprehensive care and social services to older patients throughout the area. Successful candidates will have at least two years of recent clinical experience and enjoy working with a complex and diverse geriatric population.   • Call schedule is 1 to 2 weeknights per month and 1 to 2 weekends per quarter
  • Outpatients primary care setting
  • Home visits and visits in community-based settings

  • Behavioral health and Pharmacy resources available on-site
  • Board-certification in Family Medicine or Internal Medicine is required
  • Geriatric or Palliative Care fellowship training is preferred

Providence ElderPlace is a Program of All-inclusive Care for the Elderly (PACE), and is the first program of its kind in Oregon. With eight conveniently located centers in the Portland area and on the Oregon Coast, Providence ElderPlace provides and coordinates all necessary care services for seniors to remain active within their communities. The PACE model offers medical care, physical and occupational therapy, lab tests and procedures, home care services and more.
