Sonoma Post Acute in Sonoma, CA, is seeking a passionate and motivated Assistant Director of Rehabilitation to join our team. We are looking for either a Physical Therapist Assistant (PTA) or a Certified Occupational Therapy Assistant (COTA) to help lead our rehabilitation department. Key Responsibilities:
  • Assist the Director of Rehabilitation in overseeing day-to-day operations of the rehabilitation department.
  • Lead therapy sessions and work directly with residents to achieve their rehabilitation goals.
  • Ensure compliance with all regulatory requirements and maintain documentation standards.
  • Collaborate with the interdisciplinary team to provide coordinated care.
  • Mentor and support fellow therapists and therapy assistants to ensure high-quality care.
Qualifications:
  • Licensed Physical Therapist Assistant (PTA) or Certified Occupational Therapy Assistant (COTA) in the state of California.
  • Previous experience in a skilled nursing or rehabilitation setting is preferred.
  • Strong leadership, communication, and organizational skills.
  • A passion for patient care and improving rehabilitation outcomes.
