Job Summary
A Program Specialist ensures the proper running of the entire program in accordance with Chapter 6400 and Title 55 PA Code CH 51. The Program Specialist will report to the Executive Director. You will report incidents in HCSIS and EIM and train staff. You will have the ongoing responsibility of developing and implementing provider services. This person must have a working knowledge of HCSIS and EIM systems.
Responsibilities and Duties
Specific Duties:
Completing monthly and quarterly reports.
Tracking, interpreting and analyzing participant’s goals and writing a monthly progress report based on the findings.
Attending ISP meetings and participating in the development of the ISP, and ISP annual updates.
Reviewing the ISP, completing annual updates and revisions.
Reporting content discrepancy to the SC, as applicable and plan team members.
Implementing the ISP as written.
Supervising, monitoring and evaluating services provided to the individual.
Reviewing, signing and dating the monthly documentation of a participant.
Participation in the progress toward outcomes.
Completing mandatory trainings as needed.
Maintaining compliance with Chapter 6400 and Title 55 PA Code CH 51.
Coordinating and completing assessments.
Providing the assessment as required under Chapter 6400.181 and emailing or mailing to each team member.
Reporting changes related to the individual's needs to the SC.
Reviewing the ISP with the individual and documenting.
Providing documentation of the ISP review to the SC and plan team members as required.
Informing plan team members of the option to decline the ISP review.
Recommending a revision to a service or outcome in the ISP.
Coordinating the services provided to an individual.
Entering all incidents into EIM and ensuring compliance and closing within 30 days.
Ensuring all Participant’s ISPs are current in their binders.
Qualifications and Skills
Must meet 1 of the listed qualifications:

Master's Degree with l year work experience working directly with persons with Intellectual Disabilities

Bachelor's Degree with 2 year's work experience working directly with persons with intellectual disabilities

Associate's degree or 60 credit hours and 4 years' work experience working directly with persons with intellectual disabilities.

Current Certified Investigator certificate

Current Medication Trainer through MY ODP

Current CPR & First Aid Trainer

Salary: $50,000.00 - $60,000.00 per year
